For US Passport Renewals with a change of name you will need to submit the following documents to our office.

• Most recent United State Passport
  • A passport that has expired but was issued less than 15 years ago, and you were 16 years of age or older at the time of issuance
    or
  • A valid passport due to expire within 6 months
• Documentation to show proof of name change
  • Maiden name to married name:
    Original or certified marriage certificate - from the marriage license bureau not the church (with raised seal)
    or
  • Married name to maiden name:
    Original or certified divorce decree - from the court with raised seal (must contain a clause that states you can resume your maiden name)
    or
  • Former married name to new married name:
    Original or certified divorce decree - from the court with raised seal and certified marriage certificate from the marriage license bureau
    or
    Certified marriage certificate from the marriage license bureau that shows going from former married name to new married name
    or
  • Name change due to court order:
    Original or certified court order - from the court with raised seal
• 2 Regulation Passport Photographs
  • Required size is 2x2 inches
  • You must face forward, no profiles are accepted
  • Photo background must be light or white
  • You cannot cover your head unless for religious purposes
  • If glasses are worn they cannot have a dark tint

  • Complete and sign the application
• Photocopy of valid drivers license in new name and, if available, a photocopy of social security or voter registration card in new name
• Passport Plus Authorization
  • This letter will authorize a representative of Passport Plus to submit your paperwork

• Proof of Departure
  • The Passport Office requires proof of international travel within the next 14 days.
    One of the following are acceptable :
    • Copy of airline ticket - (cannot be an open ticket)
    • Itinerary from travel agent
    • E-ticket
    • Printed confirmation from airline ticket office
    • Original company letter - you can download an outline for the company letter by clicking here
    • Faxed itinerary must come directly from the airline or travel agent and must have their name printed at the top of the fax
